양성 섬유낭성변화(FBC)와 통증 치료

 

 

유방 섬유낭성변화 (FBC)는 임신 가능 연령인 여성의 약 50%에서 발생하며, 평생 유병률(lifetime prevalence) 70~90%라는 일부 보고도 있다. FBC는 유방조직에서 느껴지는 양성 덩어리, 유방 압통, 통증, 부어오름 등의 증상을 동반할 수 있다. 이러한 증상은 월경주기 이전까지 심해지다가 월경 동안에는 가라앉는다. FBC 환자의 1/3 정도는 일주일 이상 지속되는 중등도에서 중증 주기성 유방통을 월주기로 경험한다.

 

 

Managing Symptoms

지금까지 FBC에는 danazol, tamoxifen, progesterone, bromocriptine 등을 처방하였다. 일반적으로 이러한 치료제는 견디기 힘들며 중단하게 되면 몇개월 후 증상이 다시 나타나곤 한다. 게다가 danazol은 난소기능을 억제, bromocriptine은 프로락틴 호르몬 수치를 감소시키며 항에스트로겐인 tamoxifen category D pregnancy drug이다.

 

처방이 필요없는 통증 완화 치료법으로는 온열법, 진통제, 이뇨제, 카페인 섭취 줄이기 등이 있지만 만족할 만한 효과를 주지는 못한다. 한 연구에서 비처방 치료법을 1가지 이상 사용한 환자의 50%만이 증상을 줄이는데 도움이 되었다고 답변하였다.

 

FBC의 이러한 증상들을 치료하기 위한 미국의 가이드라인이 없으므로 표준화된 치료법은 존재하지 않는다. 이에 더해 처방된 약의 부작용들로 인해 FBC 여성들은 치료를 포기하곤 한다. FBC 환자들의 요구를 만족시켜줄 새로운 옵션인 분자요오드는 요오드이온과는 다른 형태로 안전하고 효과적이다. 분자요오드는 체내에서 요오드 이온과는 다르게 행동하며 다른 효과를 나타낸다.

 

 

임상시험

Ghent와 그의 동료들은 1365명 환자를 4816woman-years (평균 추적191)치료로 분자요오드를 시험하였다. 시험 참가자의 약 2/3 (63.8%)가 폐경전, 나머지 36.2%의 환자들은 폐경 이후 여성이었다. 시험 진행 동안 통증에 객관적, 주관적 감소는 점수화되어 측정되었다. 또한 교차실험을 통해 1365명 중 75%의 여성이 증상개선과 무통증(유방)을 보고하였다.


 


요오드 분자가 FBC 증상을 감소시키는 기전

여러 연구에서 식단에 분자요오드가 부족한 경우 에스트로겐에 대한 유방세포의 민감성이 높아져 붓기, 수분 저류, 유방조직세포 수 증가 등이 관찰되었다. 요오드의 일일 권장량은 정상적인 갑상선 기능을 유지하는데는 충분하지만, 유방의 변화나 통증을 예방하는데는 부족한 것으로 보인다. 결과적으로 분자요오드가 FBC의 잠재적 치료제로 여겨질 수 있다. 분자요오드의 정기적인 사용은 에스트로겐의 세포증식효과(proliferative effects)에 대한 유방세포의 민감성을 낮춰주어 유방조직의 정상화와 FBC 관련 증상 개선을 도와준다. 이러한 작용기전에 따라 분자요오드를 이용한 FBC환자의 치료는 붓기를 가라앉히고 증상들을 줄이는데 도움을 줄 것이다.

Managing Fibrocystic Breast Changes and Pain: Perspectives for the Clinician

 

Fibrocystic breast changes(FBC) occur in approximately 50% of women of childbearing age, and some studies indicate that the lifetime prevalence of FBC may be as high as 70% to 90%. These changes may include appearance of benign lumps that can be felt in the breast tissue, as well as tenderness, aches, and swelling that occur with increased severity before the menstrual cycle and begin to subside during menstruation. This cyclic breast pain may be moderate to severe in approximately one-third of patients with FBC and last for more than 1 week of each monthly cycle.

 

Managing symptoms

Traditional management options for FBC may include prescription treatments such as danazol, tamoxifen, progesterone, and bromoscriptone. In general, these treatments are not well tolerated, and symptoms may return several months after discontinuation of treatment. Additionally, danazol may cause suppression of ovarian function, bromosriptine may reduce levels of the hormone prolactin, and the antiestrogen tamoxifen is a category D pregnancy drug.

Nonprescription pain management options include local application of heat analgesics, diuretics, and reducing caffeine intake, although these treatments typically do not provide sufficient relief. In one study, only 50% of patients who tried 1 of more nonprescription treatment options felt they were successful in reducing symptoms.

Management of FBC is currently nonstandardized, as there are no established US guidelines for treatment of symptoms of this condition. The lack of guidelines and the adverse effects of prescription medications lead many women to forgo treatment altogether. Molecular iodine, a new option for FBC that may help fill the unmet needs of these patients, has demonstrated safety and efficacy in a form unique from iodide. Unlike iodide, molecular iodine is a molecule, not a salt; as a result, it acts differently from iodide in the body and exerts different effects.

 

Clinical Study

Ghent and colleagues studied molecular iodine in 1365 women with FBC over 4813 woman-years of treatment (a mean of 191 days of follow-up). Approximately two-thirds of patients enrolled in the study were premenopausal; the remaining 36.2% of patients were postmenopausal. Over the course of the study, reductions in both objectives and subjective pain scores were measured. Additionally, in a 145-patient crossover series leading up to the 1365-patient trial, 75% of women experienced an improvement in symptoms and reported having pain-free breasts.

 

How Molecular Iodine Reduces Symptoms of FBC

Researchers observed that a lack of molecular iodine in the diet may increase the sensitivity of breast cells to the effects of estrogen, including swelling, fluid build up, and an increase in breast tissue cell numbers. While the recommended daily allowance for iodine was sufficient to maintain normal thyroid function, it appeared to be suboptimal with regard to prevention of breast changes and pain. As a result, molecular iodine is also though to help reduce sensitivity of breast cells to the proliferative effects of estrogen, resulting in normalization of breast tissue and improvement in the symptoms associated with FBC. Consistent with this mechanism of action, treatment with molecular iodine in patients with FBC reduces welling and helps reduce painful symptoms.
